Over 200 GP practices in the North East are now sharing anonymised activity data, making it easier to predict surges in demand for secondary care.
Practices are also making use of monthly reports from the primary care data quality team showing a range of information including appointments by contact type and clinical role, as well as providing comparisons against CCG area averages so that practices can compare their rate of appointments, DNAs and use of resource compares to others.
This project will provide ambulance teams in the North East with mobile access to the Directory of Services (DoS) and clinician-to-clinician video conferencing, to allow management of more patients on scene and reduce pressure on A&Es.
Remote monitoring of certain conditions, such as blood pressure or oxygen saturation can help patients manage long-term conditions proactively at home, avoiding the need for emergency healthcare.
